Electrolyte imbalances—particularly low potassium (hypokalemia) or low sodium (hyponatremia)—are among the most common concerns with long-term diuretic use and require careful monitoring.

If routine lab work reveals persistent electrolyte disturbances that are difficult to correct through supplementation alone, your physician may consider switching to a potassium-sparing diuretic such as spironolactone or triamterene.
